COVID Leave Information
THESE GUIDELINES MUST BE DONE IN A TIMELY MANNER!
VACCINATED MEMBERS PROTOCOL
- If you test positive, or are unable to report to work because of failing the Employee Self Screener (https://livepascok12fl.sharepoint.com/:w:/s/SSPS_Resources/ERAbz4_3UrRBr3nl_TD66BsBi_Ju6GHZut2MHH9XCgggGQ?rtime=udm0yNt92Ug) or are directed to quarantine by the Department of Health due to a work related COVID exposure, you should:
- Fill out the MACH form THE DAY YOU STAY/ARE SENT HOME (COVID-19 Administrative Leave effective 8-2-2021 (pasco.k12.fl.us) and submit.
- Fill out the Worker’s Comp form, available from your administrator or online. (https://www.myfloridacfo.com/Division/WC/pdf/DFS-F2-DWC-1.pdf). Turn in to your administrator and send a copy to USEP THAT DAY.
- Discuss with your administrator your ability to work remotely.
- If you are able to work remotely, nothing further needs to be done.
- If you are NOT able to work remotely, the District should be putting you on paid Administrative Leave.
- If you are not offered Administrative Leave, call our office (813.996.2119)

- Fill out the MACH form THE DAY YOU STAY/ARE SENT HOME (COVID-19 Administrative Leave effective 8-2-2021 (pasco.k12.fl.us) and submit.

- Discuss with your administrator your ability to work remotely.
- If you are able to work remotely, nothing further needs to be done.
- If you are NOT able to work remotely, the District Illness in the Line of Duty Committee will hear your case.
- You may be told to take personal leave for the time you are out.
- The ILD Committee will review your application for paid leave (up to 10 days) and render a decision.
- A favorable decision will reimburse your accrued leave for up to 10, the number of days out of work.
- If more than 10 days are needed, proper documentation must be provided to the Committee.
- The Committee will review to receive potential additional days.
